Go to MyChart. Fill out the information on the screen, including your name, address, last four digits of your Social Security number, and birth date. From there, you’ll be asked to create your own username and password. Follow the on-screen prompts to log in for the first time.

MyChart allows you to send non-urgent health care related questions to your care team — at any time — and responses are usually received within two business days. Many test results are released to MyChart right away, and you will receive a notification the moment your test results are added to your medical record.
How do I access my test results on MyChart?
Most test results are automatically released into MyChart, except certain sensitive tests. View the details of your upcoming appointments or click on a past appointment to view the after-visit summary. Outpatient progress notes are available for review as part of the visit details in the “Past Appointments” section.
