MyChart offers patients personalized and secure on-line access to portions of their medical records. It enables you to securely use the Internet to help manage and receive information about your health. With MyChart, you can use the Internet to: View your test results and health summary from the MyChart electronic health record

How do I access my MyChart test results?

How do I contact the MyChart help desk?

If you need assistance right away, please contact our MyChart Help Desk at 317-355-CARE (2273). How may we help? View health summary and test results, manage medications, request medical records and more.

What happens if my MyChart is not matching my patient record?

If the system is unable to accurately match you to your patient record you will be issued a MyChart activation code. This code may also be requested from your nurse during your hospital visit, from the Health Information Management department at the hospital, or from your Community Care Network doctor’s office.

How to use valet parking at a hospital?

A valet parking attendant will give you a claim ticket and park your vehicle in a reserved space on the hospital campus. For more information, call the Valet Office at 219-836-7319, ext. 1565.
