What is MyChart and how does it work?

MyChart is a secure, online health management tool that connects patients to portions of their medical record and members to their insurance information. MyChart is part of the electronic medical record system that is used by University of Utah Health, Carson Tahoe Health, Sublette County Rural Health Care District and Fourth Street Clinic.

How do I get access to my MyChart account?

If you’re a current patient, you have a MyChart account. To get access to your MyChart account, complete this online sign-up form. Or, call 210-450-4111 and our office will help you get access to your MyChart account. MyChart Proxy access allows a parent or legal guardian to manage a MyChart account for a child under the age of 18.

Does MyChart have direct access to my medical record?

No. MyChart offers direct access to your medical record and communicating about another individual’s information would be placed in your health record. This information would not appear in the correct health record and could potentially jeopardize medical care.

How long does MyChart expire?

For your security, MyChart activation codes and links expire after 60 days. Please contact MyChart Support at 801-213-5555 for assistance. After we verify your identity, we will send you a new activation email or text.

What is my chart ID?

Your MyChart ID is a unique username that you created when you activated your MyChart account. Some people use their email address as their MyChart ID. If you don’t remember the MyChart ID you chose, click Forgot MyChart ID? and enter your identifying information. If it matches a MyChart account in our records, your ID will be sent to the email address on file.

If you have not received a printed activation code, or an email or text for signing up, you can request one at your next clinic visit or by contacting MyChart Support at 801-213-5555. You will be asked to verify your identity before enrolling.
