How old do you have to be to have a MyChart account?

A patient 13 or older may request a personal account. Parents or guardians of minor children under 13 may request access to the minor child’s account by following the terms outlined in the “Proxy Access” section of this Terms and Conditions statement. Individuals may request proxy access to a minor’s MyChart account at their providers office. Such access will only be granted to parties with parental rights or legal guardianship over the patient and only to the extent that the party requesting proxy access can demonstrate the legal right to that patient’s medical information. Minor patients over 13 who have a personal MyChart account must request access for their parent or legal guardian to be a proxy and they do have the ability to terminate proxy access in the patient’s MyChart account settings.

How to terminate MyChart?

At any time, you may terminate your use of MyChart by notification to MyChart Customer Service at 614-366-6975 or toll-free at 866-966-6975 .

What information is in MyChart?

Information in MyChart may include treatment for physical and mental illness, alcohol and/or drug abuse, and/or AIDS (Ac¬quired Immunodeficiency Syndrome), and/or may include results of an HIV test or the fact that an HIV test was performed. Psychotherapy notes will not be released without your authorization. Through your use of MyChart you agree to the release of information to MyChart unless you terminate your use of MyChart, provided notice is received prior to the release of information to MyChart. If you share information released to MyChart, it may no longer be protected by federal privacy rules, such as HIPAA.

What is the purpose of MyChart?

The purpose of MyChart is to provide you with (1) access to selected portions of your medical records, such as basic laboratory results, summary medical history, visit history, and selected billing information, and (2) the ability to communicate with your health care provider.

Can OSU Medical Center terminate my chart?

Your access to MyChart continues from the date of activation until terminated by OSU Medical Center or by you. OSU Medical Center reserves the right to terminate or restrict your ability to use all or part of MyChart for misuse, abuse, inactivity or for clinical reasons as determined at the sole discretion of OSU Medical Center. If your access is terminated or restricted, then you will be notified via e-mail and U.S. Postal Service.
