Is MyChart required to receive health care from Lexington Medical Center?

The use of MyChart is completely voluntary and is not required to receive health care from Lexington Medical Center. I. Eligibility Individuals must be age 16 or older to request a personal MyChart Account.

What is MyChart?

MyChart gives you secure, online access to your medical record anywhere, anytime 24/7. With your MyChart account, you can: Manage your appointments: Schedule your next appointment or view details of your past and upcoming appointments. Access your test results: No more waiting for a phone call or letter.

Is MyChart my complete medical record?

MyChart is not your complete medical record. MyChart provides you with access to some (but not all) of the information contained in your electronic medical record. As a result, you should not rely on information accessed via MyChart as complete.

What happens if I send a message through MyChart?

All electronic messages you send through MyChart will become part of your Lexington Medical Center medical record and may be used or disclosed as provided in this Agreement and the Notice of Privacy Practices.

How long does a proxy account last in LMC?

For access to the LMC MyChart Account of an adult patient or a minor patient ages 16-17, the patient and the proposed proxy complete the “Proxy for Adult Application.” Proxy access to an adult patient’s MyChart Account will automatically terminate three years after establishing proxy access.

How old do you have to be to get a MyChart account?

Individuals must be age 16 or older to request a personal MyChart Account. Parents or guardians of minor patients, patient representatives, or other individuals may request access to a patient’s MyChart Account by following the terms outlined in the “Proxy Access” section of this Terms and Conditions statement.
