How do I sign up for MyChart?
MyChart – signup page Please Identify Yourself All fields are required. MyChart activation code Enter your activation code as it appears on your enrollment letter or After Visit Summary®. Your code is not case sensitive.
How can I access my covenant medical records?
Please check with your physician’s office to see how you can access MyChart. MyChart is not to be used for urgent medical needs. For medical emergencies, dial 911. Patients who have been admitted to Covenant may also set up access to MyChart to view their Covenant medical records.
How can I access the MyChart app on my Android or Apple phone or tablet?
A member of the support team is ready to help you register. To access the MyChart app on your Android or Apple phone or tablet, simply go to your App Store and search for MyChart. When you download the app, select Covenant Health from the list of organizations and log in with your MyChart username and password.

To access and prepare for your scheduled virtual visit: Virtual visits are accessed through your MyChart account.
