Allows an individual to have full access to the myChart account of an adult 18 years of age or older. Access is obtained by the patient inviting the proxy online from within his/her myChart account or in clinic where both complete the myChart Proxy Form and providing it to the patient’s Care Provider.

How to access MyCarle account for a 17 year old?

In order for a parent or legal guardian to access the MyCarle account of a 12 to 17-year-old patient, the patient will need to go into the doctor’s office and sign a consent form. Once the patient signs the form, the parent or legal guardian will be able to see the same information the patient sees in his/her MyCarle account. The patient has the right to revoke this access at any time in compliance with state of Illinois regulations.
