Who can access the MyChart® patient portal?

Welcome to the MyChart® patient portal. Please read these Terms and Conditions carefully before accessing and using MyChart. This instance of MyChart is available to patients who are participating in the Epic Community Connect Program (the “Program”).

Who owns the MyChart service?

I acknowledge and agree that Parkland and its licensors (including Epic) own all proprietary rights to the MyChart service and all technology related thereto.

How do I open a MyChart account?

In order to open a MyChart Account, each user must comply with the Program’s and Providers’ then-current policies. In any event, I understand that my enrollment is contingent on verification of my identity either in person by a Program or Provider employee.

Is MyChart an emergency system?

PLEASE NOTE: MYCHART IS NOT AN EMERGENCY SYSTEM. The MyChart system should never be used for urgent matters. The anticipated turnaround time for response to electronic messages is 1 to 2 business days, but could be longer.

I acknowledge and agree that Parkland and its licensors own all proprietary rights to the MyChart service and all technology related thereto. I agree not to copy, modify, create derivative works of, publicly display or perform, republish, transmit, distribute or reverse engineer any of Parkland’s or its licensors’ proprietary material. Except for my permitted use hereunder, I acknowledge that I have no right, title or license to any of Parkland’s or its licensors’ intellectual property.
