How do I access MyChart?

If you have questions about what’s available, contact your healthcare organization. To access MyChart, you must create an account with your healthcare organization. To sign up for an account, download the app and search for your healthcare organization or go to your healthcare organization’s MyChart website.

How do I access my Child’s charts?

Download the “MyChart” app on your phone or tablet. Indicate the state they are in, and then select “PPOC” from the list of providers using MyChart. *Pediatric & Adolescent Medicine will not show up in the list of providers, so look for PPOC. Parents can access all of their children’s charts with 1 login (all children under 13 years)
