What is the OSF MyChart bedside experience?
With the OSF MyChart Bedside experience, your medical information will be updated in real time when you’re admitted to a participating hospital. By using a personal mobile device with the MyChart app enabled, you (and your loved ones with proxy access) will be able to access up-to-date information that’s shared to your OSF MyChart account.

The eCheck-In feature is available at most OSF HealthCare locations, starting four days before your scheduled appointment. It can be used for your provider appointments and hospital outpatient appointments, such as rehab, diagnostic tests and more. eCheck-In is required for all video visits.
How to obtain adult proxy?
Download and print out an Adult Proxy Form that you must complete to request access to the records of an adult who is incapacitated or unable to make decisions. Present the completed form to the patient’s provider or the hospital.
