Sign in here with your Gundersen Health MyChart username and password. Don’t have an existing Gundersen Health MyChart account?

How do I sign up for Gundersen Health?

Review your health records, including notes, medications, allergies and immunizations. Do all the above for family members, dependents (age 18 or older) or friends who give their permission. Step 1: Go to mychart.gundersenhealth.org and click Sign Up Now Step 2: Enter the activation code, if you have one.

What can I do with MyChart?

With MyChart, you can access and manage your health information 24/7. Sign up anytime! Communicate with your care team and get answers to your medical questions. Request or cancel appointments, and view details for past and future appointments. Begin an On-Demand Video Visit or E-Visit for urgent and non-urgent care, respectively.
