What is MyChart?
CommUnityCare Health Centers is proud to introduce a new medical record system that will bring many new benefits to you, the patient. Part of this system is an application called MyChart. This is an easy-to-use secure application that gives you access to your health information.

How to setup MyChart account?
Go to the App Store or Google Play and download MyChart, click or scan the QR code below, or go to MyChart.com to setup an account.
How long does my chart activation code last?
For your security, your activation code expires after
30 days and is no longer valid after the first time you use it. If you still have problems, email us at
[email protected] .com or you can call our MyChart Patient Support Line at 512-978-8884.
How long does it take to get a response from MyChart?
You will generally receive an answer within 1-3 business days. Please note that MyChart should not be used for urgent situations. Please contact your clinic if the situation requires immediate attention or dial 911 if it is an emergency.
How long does it take for a blood test to be released?
Your test results are released to your MyChart account after your Dr. has reviewed them. This is generally within 1-12 days.
