How do I enter my wakehealth activation code?

All fields are required. myWakeHealth Activation Code Enter your activation code as it appears on your After Visit Summary (your code is not case sensitive). The code is no longer needed after you complete this process. Activation Code Part 1

A parent/legal guardian needing access to a minor child. A parent/legal guardian will have limited proxy access for children ages 12 to 17. On a child’s 12th birthday, access to an account will automatically change to the limited view. See question below.

How long does my wakehealth activation code last?

You will use the activation code only once to sign in to myWakeHealth for the first time. (The code will expire after you have used it or after 60 days). When you sign in to myWakeHealth the first time, you will enter your activation code and then be asked to create your own unique myWakeHealth ID and password.

How long does a code expire?

Your activation code expires after you successfully sign up or 60 days after you received the code. If you have not signed up within that time frame, you can still sign up online without a code.
