How do I get a MyChart activation code?
At your doctor’s office: All Contra Costa Regional Medical Center & Health Centers patients receive a unique MyChart activation code in the “After Visit Summary” paperwork given to them following an office visit. Go to the MyChart website and follow the step-by-step sign-up instructions in your After Visit Summary.

MyChart will be unavailable on Saturday November 19th from 12 am to 3:00 am due to system maintenance. Do I need to be a Contra Costa County resident?
You do not need to be a Contra Costa County resident. We will email confirmation of your appointment. Please check your spam or junk folder if you do not immediately receive an email. Patients who are younger than 18 will receive Pfizer vaccine.
How old do you have to be to get a booster shot for Johnson and Johnson?
Johnson & Johnson: Anyone 18 or older who received a Johnson & Johnson vaccine may receive a booster dose of Johnson & Johnson, Moderna or Pfizer vaccine if at least two months have passed since they were first vaccinated.
What age group is at high risk of serious illness?
People who are 18 or older and have underlying health conditions that put them at high risk of serious illness if infected
Can you get booster shots with Pfizer?
People who were earlier vaccinated with Moderna or Pfizer may receive booster dos es of Johnson & Johnson, Moderna or P fizer.
Do you have to be a resident to make an appointment in Contra Costa County?
Anyone who is 5 or older is welcome to make an appointment at one of our sites. You do not need to be a Contra Costa County resident.
