What is MyChart and how can I use it?

This online tool allows you to view your medical records, appointments and more to interact and stay up-to-date with your health. Sign up for or log into MyChart on the right, and talk to your physician about how Southcoast Health can partner with you to achieve a happy, healthy life.

How do I Activate my MyChart account?

We need some information in order to grant you a MyChart account. Enter your demographics here and in the next step we will verify your identity using questions from a third-party verification system. Once verified, you will receive an email or a letter with your activation code and instructions on how to activate your MyChart account.

How do I view scheduled appointments in MyChart?

Once logged into MyChart, select the Visits icon which can be found at the top of the screen. Scheduled appointments will be listed on this screen and the patient can select the details button to the right-hand side.

How to view my child’s medical records?

You can sign up to view your child’s medical records through MyChart by completing a MyChart Parental/Guardian Access Children Under 12 Application form. A separate application is required for each child. However, both parents may request access that that child on the same application.
