How do I sign up for MyChart?

Do minors get a MyChart activation code?
Yes, minors (13-17) will be given a MyChart activation code at their next office visit to create their own MyChart account. Access codes will only be given to the teen to create their account.
Who can see my medical records in MyChart?
You are the only one able to see your medical documents in MyChart, unless you choose to share your records with a family member, caregiver or loved one. For more information on granting someone else access to your account, please log into your MyChart account, go to “Health”, click on “Share My Record” and click on “Friends and family access.”
How do I access my St Luke’s MyChart?
Yes, you can access your MyChart from our mobile website or through our mobile app. Visit sluhn.org/MyChart on the web, or visit the Apple Store or Google Play Store to download our St. Luke’s mobile app! What to do if you don’t have access code?
If you don’t have an access code, you can request an account online and you’ll receive an activation email.
Can you telemedicine with MyChart?
Patients enrolled in MyChart can also take advantage of our telemedicine options, including eVisits and video visits.
Is UConn Health video scheduled in advance?
Please note: UConn Health video visits are scheduled in advance and are not for emergency care. If you have an emergency, dial 911.
How old do you have to be to use MyChart?
Parents and legal guardians can request proxy access to their child’s MyChart information until the child is 18 years old. The information available to the child proxy through MyChart is limited for children aged 13-17.
