How do I login to MyChart?
Go to MyChart. Fill out the information on the screen, including your name, address, last four digits of your Social Security number, and birth date. From there, you’ll be asked to create your own username and password. Follow the on-screen prompts to log in for the first time.

How to share health information with MyChart?
To permit information sharing: Choose an app you wish to share your health info with OR respond to an app requesting you to share your health info. Read the app’s Terms & Conditions. Follow the app’s instructions. At the MyChart login screen, enter your user ID and password to allow health information sharing with the app.
How to get activation code for MyChart?
Access the MyChart login page and click Sign Up Now. On the Please Identify Yourself page, click Request Online. The Activation Code page will appear, enter your demographics information.
How to authorize proxy access?
To authorize Proxy access, bring the completed MyChart Proxy Access Authorization Form, proper identification, and any additional required documentation to your provider’s office or any MetroHealth System clinic. Additional information may be requested. A staff member will review the form and verify information regarding the patient and parent or guardian. All items must be submitted in person with a valid government-issued photo ID in order to verify identity.
How long does it take for a MyChart test to be released?
Your test results are released to your MyChart account after your provider has reviewed them. Many test results are available within 3 days. Please note that some test results may be released before your provider can review them. In this case, always check the “notes” section for updates from your provider about values, concerns, etc. If you do not see a result in MyChart that you are looking for or if you have questions about a result, contact your clinic directly.
