How do I create a MyChart account at UVM Medical Center?

Visit UVMHealth.org/MyChartSignUp to complete an online sign-up form. If you have an activation code from your provider’s offi­ce, a recent bill, or a printed After Visit Summary, visit UVMHealth.org/MyChartCode and enter it. Skip to step 4. You do not need to create a new MyChart account if you had a UVM Medical Center MyHealth Online account.

What is MyChart?

MyChart is the online patient portal associated with Epic, the electronic health record (EHR) used by the UVM Health Network. An EHR is a real-time, digital version of a patient’s medical record and can be viewed and managed by authorized health providers.
