How do I access myfhn?

Choose the FHN/MyFHN Portal and download it – it will now appear as an icon on your smartphone. Anytime you want to access MyFHN through the app, just log in with the same username and password set up for the online version of MyFHN.

What is MyChart?

When will my test results be available in MyChart?

Your test results are released to your MyChart account after they have been sent to your doctor for review. They may be available immediately, or within approximately 7 days. If you don’t see your results after this time, please contact your doctor’s office.

Who can access my Child’s Health Information in MyChart?

MyChart is available to children age 13 and older. If you have a child under 13 years old, a parent or legal guardian may access their health information in MyChart by becoming their proxy. To request family/proxy access to your child’s health information, you must complete a family access form.

What is a proxy authorization form?

A Proxy Authorization Form is required to access the health information of anyone over age 12 by someone other than themselves. Please note that all signatures must be witnessed by an FHN staff member. If you have any questions about this process, please contact the MyFHN Coordinator at 815-599-6907 or toll-free at 1-877-6000-FHN (1-877-600-0346) ext. 907. To revoke proxy permission, a Proxy Revocation form must be completed.
