ANY CLAIM RELATED TO SPECTRUM HEALTH MYCHART MUST BE INITIATED WITHIN ONE (1) YEAR OF THE DATE OF THE EVENT THAT GAVE RISE TO THE CLAIM. EXCEPT IN THE CASE OF PROPERTY DAMAGE OR BODILY INJURY, INCLUDING DEATH, SPECTRUM HEALTH’S LIABILITY IN ALL OTHER EVENTS IS LIMITED TO FIVE DOLLARS.
