How do I sign up for MyChart?
Once you click on “CLICK HERE TO BEGIN” above, select “SIGN UP NOW” on the right side of the page If you do not have Activation Code, click “SIGN UP ONLINE” on right side of the page Set up username and password (Email addresses can only be associated with one MyChart account.
Can a 12-17 year old use MyChart?
Patients with ages 12-17 will be unable to create/access MyChart (This is an automatic equation built into MyChart and is something Nor-Lea Hospital District is unable to control.) If you have a shared email address with a spouse or family member, the email address should only be used in association with one MyChart account.

Can I create a MyChart account if I don’t have a Social Security number?
Make sure to provide your full legal name, date of birth and email address in the body of your inquiry. Patients who do not have a social security number in our Electronic Medical Record system will be unable to create a MyChart account. Please have that number available when you contact.
