What are the MyChart® patient portal terms and conditions of use?

MyChart® Patient Portal Terms and Conditions of Use Lexington Medical Center is pleased to provide the web site MyChart® to allow patients and appointed patient representatives, called “proxies,” to access certain portions of the patient’s medical record. Never use MyChart for emergency or other urgent medical issues.

Is MyChart a substitute for a doctor’s visit?

All health and health-related information in MyChart is intended to be general in nature, provided solely for patient convenience, and is not a substitute for a visit to a licensed healthcare provider. Please see your doctor, not MyChart, for all medical advice and care.

Is MyChart required to receive health care from Lexington Medical Center?

The use of MyChart is completely voluntary and is not required to receive health care from Lexington Medical Center. I. Eligibility Individuals must be age 16 or older to request a personal MyChart Account.

How old do you have to be to have a Connect account?

Any patient age 12 or over is eligible for a Connect account. In addition, parents can be given portal proxy access to manage their children’s accounts, or patients can grant access to their own accounts to family members, friends, or caregivers. How long is a self sign up good for?

If you get an activation message via text message or email, you have 24 hours to respond to it. If you get a letter with a code on it, it is good for 30 days. Also, you can complete the Self Sign Up process without an activation code, and the system will ask you questions to verify your identity.
