How do I get an activation code for MyChart?
Click the “Login to MyChart” button above, which will take you to the OCHIN* MyChart home page to request an activation code. Click “Sign Up Now.” Complete the MyChart Request an Activation Code form and select “Submit.” A one-time activation code will be emailed or mailed to you.
What to do if you don’t have refills?
If you have refills remaining, please contact your pharmacy directly. If you do not have any refills and need a new prescription from your provider, you can quickly & easily request them
