How do I schedule an appointment at CHI Health Clinic?
Open scheduling is available online with select providers. Use our patient portals to request your appointment online with select providers. Chat to schedule is available for CHI Health Clinic Primary Care providers 7 days a week from 8 a.m. – 5 p.m. Need an unplanned visit? We have options for you including Virtual Quick Care and Priority Care.
How do I ask for health information other than the patient portal?
To ask for health information that is not in your Patient Portal, and/or to ask for health information (including EHI) in a manner other than through the Patient Portal, please contact the Health Information Management (HIM) department of the facility where you received care.
