WellnessPointe is improving our patient experience! With our new PatientPortal, you can access your health information easily online from wherever you are, all in one place.
How do I access the wellness portal?
Log on to your account at sanfordhealthplan.com/memberlogin. Under the Insurance tab, select Portals and Links, then Wellness Portal. If you do not have an account, click the “Request Access for Yourself” button. Contact Customer Service at (800) 752-5863 with questions.
What services does wellness Pointe offer?
In addition to medical, dental, counseling, and vision services, Wellness Pointe has a variety of grant-funded programs to augment your family’s health and social needs. From parenting education to youth substance prevention programs, our social services programs are designed to keep your family strong, healthy, and happy for years to come.
What is the patient portal?
This interactive web portal empowers all Community Hospital patients and their families to take a more active role in their care by providing easy, secure access to health information and online communication with Patient Portal staff. All of the information in the Patient Portal comes from your Community Hospital Electronic Health Record.
