Go to MyChart. Fill out the information on the screen, including your name, address, last four digits of your Social Security number, and birth date. From there, you’ll be asked to create your own username and password. Follow the on-screen prompts to log in for the first time.

Many test results are released to MyChart right away, and you will receive a notification the moment your test results are added to your medical record.
