Go to MyChart. Input your activation code, last four digits of your Social Security number, and birth date. From there, you’ll be asked to create your own username and password. Follow the on-screen prompts to log in for the first time. If you did not receive an activation code, no problem.

What is a unique username and password?
A. Unique username and password. Your username and password control access to your MyChart account and your health information within the account. DO NOT share your username or password with anyone. Sharing your username or password could compromise the confidentiality and security of your information in MyChart, and may result in AHN suspending or terminating your access privileges.
What information does AHN collect?
AHN collects personal information about you, such as name, email address, phone number, and other demographics, for MyChart account administration purposes. We also collect other information, such as geolocation (if enabled on your personal device), in order to deliver certain MyChart programs which are driven by your proximity to an AHN facility.
