At the Diamond Headache Clinic, MyChart access allows a patient to: Pediatric (18 years or younger) proxy access for MyChart allows the parent (s) or guardian (s) to communicate and have access to the pediatric patient’s Electronic Medical Record (EMR). Access is obtained by completing the PATIENT PORTAL PROXY ACCESS REQUEST AND AUTHORIZATION.

What does acceptance of my MyChart proxy mean?

Your acceptance indicates that you have read, understand, and agree to these Terms and Conditions of Use. If you are acting as a MyChart Proxy for another individual, these Terms and Conditions shall apply to your proxy access in the same manner as they apply to your own MyChart access.
