How to get proxy access for a 13 year old?
In order for you to gain proxy access for a teenager (aged 13-17), the parent (s) or legal guardian (s) must first complete the myChart at Cape Fear Valley MyChart Proxy Access Request Form and Authorization for Teenaged Minor form. Additionally, the teenager must sign a Proxy Access Authorization Form for Release of Information. Once these forms are completed they can be returned to the teen’s provider clinic or medical records for an invitation to be sent to the proxy.
