The new Patient Portal provides convenient, secure online access to the Physician Group of Capital Health Plan, allowing members to maintain an ongoing relationship with their doctor and practice staff. What information should be included in a capital health portal?
The information in the portal should reflect your name, address, phone number, and insurance information that were current at the time you were at Capital Health for your test, Emergency Department visit, or inpatient stay. Any changes in name, address, phone number, insurance information will be updated at your next visit to Capital Health.
How do I access the hospital portal?
Select “Doctors Office Portal” for any provider office visits (even if the office is located at the hospital).* Select “Hospital Portal” for any information related to a hospital stay or ER visit as a patient or to view direct results related to testing done at the hospital or our Capital Health – Hamilton testing site.**
