How often do you need a well visit?

For older children and teenagers, well visits are required every 12 months if you need a provider signature on any forms – such as school health forms, camp forms, or sports forms. Follow-up visits are also required every 3 months if your child is taking any on-going prescription medication from our office.

What is a well child visit?

Well-child visits are an integral part of keeping your kids healthy and track their growth and development. They allow our providers to get to know you and your child, monitor any health or developmental concerns, answer your questions, and give you feedback and advice.

How long does it take to get immunization records for camp?

Camp/School forms require 2-3 business days to complete. Generally, immunization records can be available within 1 business day of your request. However, we will not be able to sign off on your forms unless your child is up-to-date on his/her vaccines and well visits (within the last 12 months).
