Welcome to MyCHArt, the CHA patient portal. MyCHArt lets you manage your health care and access your electronic health record (EHR) from your computer or mobile device. How do I? Get a quick view of the number of vaccine doses you received, the manufacturer, lot number, and location of your vaccine (s).
How do I request family access to MyChart?
Request family access for your children or others in your care by contacting MyChart Support at 770-219-1963 or 855-208-0122. Need some help? If you need help accessing MyChart, contact the MyChart Support HelpDesk at 770-219-1963, Monday through Friday, from 8:00am to 4:30pm.

How can I view providers’ notes in MyChart?
To help you feel more involved in your child’s care, we have added a feature in MYchart called Open Notes. Through Open Notes, you can now view providers’ notes in MYchart. Notes often cover information about diagnoses, tests, procedures, medicines and treatment plans. The timing for when you can view notes depends upon the care setting.
