    What is MyChart and how does it work?

    MyChart is an online tool that connects you with your personal health information and your healthcare team quickly and easily. Any care you receive from a Prisma Health provider will appear in your MyChart account online.

    Connect is a joint effort of Columbia, NewYork-Presbyterian, and Weill Cornell Medicine, so a single Connect account links you to all three institutions. With the Columbia Connect app, you can access convenient services to help manage your health. Learn more about the Columbia Connect app.

    How do I continue to connect with my Columbia providers?

    To continue to connect with your Columbia providers, sign up for a new Connect account. Your new account will not have all of your health information from your FollowMyHealth account, but after our February launch, your health information will begin to flow to your new Connect account.

    How do I access my columbiadoctors Records?

    And now, ColumbiaDoctors records are available to you at any time of day—or night.     How old do you have to be to have a Connect account?

    Any patient age 12 or over is eligible for a Connect account. In addition, parents can be given portal proxy access to manage their children’s accounts, or patients can grant access to their own accounts to family members, friends, or caregivers.     How long is a self sign up good for?

    If you get an activation message via text message or email, you have 24 hours to respond to it. If you get a letter with a code on it, it is good for 30 days. Also, you can complete the Self Sign Up process without an activation code, and the system will ask you questions to verify your identity.

    Can an adult access another adult’s account?

    Any adult can be given access to another adult’s account, by granting portal proxy access. The portal proxy does not need to be a patient. The patient will need to complete the Connect Patient Portal Proxy Access Authorization form.

    Can you review your medical history on Connect?

    Our teams are bringing key components of your current medical history into Connect, which you can review with your provider. Rest assured that your doctors will have access to your full record.
