What is MyChart and how does it work?
MyChart is a free service offered to Community Health Network patients. It offers patients personalized and secure online access to portions of their medical records. It enables you to securely use the Internet to help manage and receive information about your health. With MyChart, you can use your desktop or mobile device to:
What happens if my MyChart is not matching my patient record?
If the system is unable to accurately match you to your patient record you will be issued a MyChart activation code. This code may also be requested from your nurse during your hospital visit, from the Health Information Management department at the hospital, or from your Community Care Network doctor’s office.
How do I contact MyChart patient support?
You can also contact our MyChart Patient Support Line at 219-226-2313to request assistance. How do I create a MyChart account?
Patients who wish to create a MyChart account will be issued an activation code during their clinic visit. This code enables you to log in and create your own username and password.
