How do I get a MyChart activation code?
At your doctor’s office: All Contra Costa Regional Medical Center & Health Centers patients receive a unique MyChart activation code in the “After Visit Summary” paperwork given to them following an office visit. Go to the MyChart website and follow the step-by-step sign-up instructions in your After Visit Summary.
Where can I get free tests in Contra Costa County?
For Kaiser members: Kaiser members can order up to 8 at-home tests a month directly from Kaiser at kp.org. People with other health coverage besides Kaiser can also request free home test kits through their providers. Contra Costa County Libraries: Free test kits are available to county residents at all 26 county-run library branches.
Do I need to be a Contra Costa County resident?
You do not need to be a Contra Costa County resident. We will email confirmation of your appointment. Please check your spam or junk folder if you do not immediately receive an email. Patients who are younger than 18 will receive Pfizer vaccine.

How old do you have to be to get a booster shot for Johnson and Johnson?
Johnson & Johnson: Anyone 18 or older who received a Johnson & Johnson vaccine may receive a booster dose of Johnson & Johnson, Moderna or Pfizer vaccine if at least two months have passed since they were first vaccinated.
