Does DMOS offer outpatient surgery?
Outpatient convenience. Currently DMOS offers two outpatient surgery centers. One in Ankeny at the Ankeny Medical Park Surgery Center (AMPSC) and the other is located in West Des Moines at the Orthopaedic Outpatient Surgery Center (OOSC). Each are joint ventures with our partners at UnityPoint Health-Des Moines.
How do I file a medical record request for DMOs orthopaedic centers?
Please ensure the form is filled completely. Once completed, please mail it the form to DMOS Orthopaedic Centers c/o Medical Records 6001 Westown Parkway, West Des Moines, Iowa, 50266. Patients may also fax the completed request to 515.224.5337. Mailed/Faxed requests take 7-10 business days from the receipt date to be processed.
How do I contact DMOS?
Any further questions AFTER speaking with your insurance company can be emailed to [email protected] or call (515) 224-1414 Option 7. Past Due Accounts: Account balances that have gone unpaid greater than 90 days without payment in FULL may be sent to a collection agency for payment collection.
What should I bring to DMOS?
It is always a good idea to bring a list of all medications you are currently taking, along with a list of any medical conditions you might have. Please note, there is not a cafeteria at DMOS, so if you require frequent nourishment, remember to bring a snack.
