How do I sign up for MyChart?
To sign up for MyChart, call 888-243-7153 . Requests should be sent from your insurance company, attorney, or Disability Determination Service (DDS) and mailed to the address on the authorization form. All charges for medical records will be billed to the requestor and in accordance with the State of New Hampshire Statutes.
What browsers are supported by MyChart?
MyChart is the Patient Portal for Elliot Health System and Southern New Hampshire Health System. MyChart is ending support for Internet Explorer on 12/6/21. Please switch to a supported browser, you can open MyChart with Google Chrome, Mozilla Firefox, or Microsoft Edge for an optimal user experience.
Can I access my family member’s MyChart record?
Yes, this is called “proxy” access. For a child under 12, this allows a legal parent or guardian to log into their personal MyChart account, and then connect to information regarding their child.

For many years, Elliot Health System patients have had secure online access to their electronic medical records through the MyEChart patient portal. Moving forward, MyEChart will be called SolutionHealth MyChart and will be available at both Elliot Health System and Southern New Hampshire Health, founding members of SolutionHealth.
How does MyChart work?
MyChart gives you the ability to access your health information at your convenience, through a secure website. MyChart allows you to be actively involved in your health care, the ability to communicate with your providers, and help you stay informed. Some of your medical records are already available to you online. As a registered user, you can review, download, and save to your computer or print the following information: 1 Immunization record 2 Downloadable Clinical Health Summary 3 Hospital Summary 4 Past office visits 5 Prescription medications 6 Most lab test results 7 Your current health summary, including any allergies and ongoing health conditions
How to get a copy of my health records?
You may request a copy of your records on paper, on USB flash drive, on a compact disc (CD) or through MyChart. A completed and signed Authorization to Release Protected Health Information form along with valid identification is required for copies of records to be released. To request the form be faxed or mailed to you, please call (603) 663-2341.
