    Once you have an account, visit ExcelaHealth.me or the FMH Mobile App, sign-in anytime and communicate about your personal health information! To access your records on the go, check out the FollowMyHealth (FMH) smart phone app. You can download the FMH app from the Apple or Android App store.

    Why is my FollowMyHealth app not working?
