What is MyChart?
Welcome to MyCHArt, the CHA patient portal. MyCHArt lets you manage your health care and access your electronic health record (EHR) from your computer or mobile device. How do I? Get a quick view of the number of vaccine doses you received, the manufacturer, lot number, and location of your vaccine (s).
Does MyChart have direct access to my health record?
NO. MyCHArt offers direct access to your personal health record and communicating about another individual’s information would be placed in your health record. This information would not appear in the correct health record and could potentially jeopardize medical care.
Can a parent access my MyChart account?
Yes you can. This is called Proxy access and allows a parent (or guardian) to log into their personal MyCHArt account, and then connect to information regarding their family member. Request this with the patient’s provider’s office and complete a Proxy Consent Form to obtain access to this convenient service.
How do I update my MyChart account?
Log into MyCHArt and from the left menu, go to the My Account section and select the appropriate option under preferences. Patients can only update email, security question and password.
