Does horizon eye care have a patient portal?
Our knowledge and your lifestyle ensure you get the best vision possible! Horizon Eye Care now has a new patient portal. A secure way of electronically communicating with our staff as well as access to your patient health record. On average, we recommend scheduling an appointment once every two years to ensure your eyes are remaining healthy.

Does Horizon Family Medicine offer same-day urgent appointments?
Since as a clinic we serve over 18,000 panelled patients, Horizon Family Medicine is an extremely busy practice. However, we do strive to provide same-day urgent appointments when required, and offer Urgent Access availability to our patients (similar to a “walk-in” for Horizon patients).

Go to MyFamilyHealth.org and click on Patient Portal. Click on “Log In” under “Already Enrolled”. Under the user name and password field is a link…. “Need help with your user name and Password?” Click on this.
