How do I create a MyChart account with Cleveland Clinic Martin health?
Please contact 772-210-4228 to establish with Cleveland Clinic Martin Health. Once established you can complete self-signup to create a MyChart account. Thank you for requesting an activation code online.
How do I Activate my MyChart account?
We need some information in order to grant you a MyChart account. Enter your demographics here and in the next step we will verify your identity using questions from a third-party verification system. Once verified, you will receive an email or a letter with your activation code and instructions on how to activate your MyChart account.

How to login to mychart?
To login to MyChart, visit mychart.baptisthealth.com. Once on the page, enter your username and password that you setup when you created your account, and click Sign In.
What is eCheck in?
The “eCheck-In” activity will ask you to review and verify your personal information, just as you would if you were registering with the front desk. You will then be prompted to complete any medical questionnaires related to your visit. If at any time you cannot complete part of the process, you can cancel or tap “Finish Later” to leave the activity and save your answers. If you come back later to continue, you’ll be returned to where you left off.
How long does it take to get a response from a medical question?
