What is MyChart and how does it work?
MyChart offers patients personalized and secure on-line access to portions of their medical records. It enables you to manage and receive information about your health securely via the Internet. With MyChart, you can use your online MyChart account to: *Request medical appointments.
How do I gain access to another MyChart user’s account?
To gain access to another MyChart user’s (age 14 and older) account, ask them to send you an invite to view their MyChart account. They would log into MyChart using MyChart.OhioHealth.com, select ‘Share My Record’ under the Health menu, select ‘Friends and family access’ and ‘Invite Someone’.
How do I use my MyChart activation code?

What do you need to request a copy of a child’s record?
If you are requesting a copy of another person’s record, you will need to provide legal documentation verifying legal guardianship, power of attorney for healthcare, executorship or next-of-kin relationship of a decedent. Parents may request copies of their minor child’s record if they have legal custody and the child is not legally emancipated.
How long does it take to get continuity of care records?
Please also include which care site or sites records are needed from. Turnaround time for continuity of care requests is normally three business days or less.
Do you pay for a copy of your medical record?
If requested for follow-up care, there are no fees for a copy of your medical record. There may be a fee when requested for other purposes (such as legal, insurance claims or personal use).
