MyChart is a free, secure website where Institute for Family Health patients can see their medical record, make appointments, and more on any device. For more information, read our MyChart FAQs. To read about how we updated MyChart in 2020, click here. The signup process is quick and simple.
How do I access my child’s MyChart?
Complete pre-visit questionnaires, which can then be reviewed by your or your child’s doctor, and imported right into your or your child’s official medical record at the time of your visit. Grant access to MyChart to other adults (with appropriate consent).
What if I can’t view my account in MyChart?
For questions regarding an account that you are unable to view in MyChart please contact the customer service department at 801-587-6303 or 800-862-4937.
