MyChart can be accessed via the MyChart website or mobile app (available for download from the iTunes App Store and Google Play ). If your child is 17 years old or younger or you are 13 years old or older, you may now go online to request a MyChart account. Simply select the form that best describes you.

As a MyHealth user, your role in maintaining the security of your medical information is: 1) changing your password on a regular basis, and 2) keeping your login ID and password confidential.
What is Legacy Health?
Security and Confidentiality – Legacy Health is committed to protecting the confidentiality of your medical information. We afford the same degree of confidentiality to medical information stored in MyHealth as is given to medical information stored by Legacy Health in any other medium. We limit Legacy Health employees’ access and ability to enter or view information based upon their role in your care. Firewalls, passwords, encryption, and audit trails are further used to safeguard your information. We keep a record of all access to MyHealth, including the date and time of access by a patient, proxy, or Legacy Health staff. We will take all appropriate steps to ensure information received from our online visitors is as secure as possible against unauthorized access and use.
How old do you have to be to get a MyHealth account?
Minor Eligibility – You may request a MyHealth account if you are 14 years of age or older. Parents or guardians of minor children may request a MyHealth account on behalf of their minor children as well as access to their minor child’s account by following the terms outlined in the “Proxy Access” section below.
