How do I contact Valley Health MyChart support?
For additional assistance please call Valley Health MyChart Support, 888-730-4812 . Access MyChart on the go by installing the MyChart App on your smartphone or tablet. Search for the MyChart application in the Apple or Android app store. When the results appear, select the MyChart Epic systems app .
How do I Activate my MyChart account?
The staff will assist you in signing up for a MyChart account during your visit at one of our locations. Click on the Activate Your Account button above. Use the activation code that can be found on your enrollment letter, bill or the last page of your visit summary. Follow the instructions to activate your account.
What should I do with MyChart?
MyChart should be used for keeping track of your medical history, documents, setting up appointments and other non-urgent medical needs. If you have an urgent medical issue, please call your doctor’s office rather than using MyChart. If you have a medical emergency, dial 911.

If you need copies of your records, complete and sign a Authorization to Use or Disclose Protected Health Information Form.
