What is MyChart?

MyChart is an online service offered by Yale New Haven Health that allows you to connect with your doctor’s office, schedule appointments, request prescription renewals, review your health history and more — any time! All you need for access is an activation code from your doctor and your date of birth.

How can I get an activation code for MyChart?

Go to the MyChart Request an Activation Code page to get your activation code. Or you can ask the receptionist at your next visit to email you a code or go to the Member Services desk and ask to be emailed a code. Or after your visit you will receive an After Visit Summary.

How long does it take to get a MyChart account?

We need some information from you in order to grant you a MyChart account. Once submitted, it could take up to a week for processing and verification. Once approved, you will receive an email or a letter with your activation code and instructions on how to activate your MyChart account. If you have any questions, please contact your clinic.

If you have proxy access for your child, you will be able to message their primary care clinician in Pediatrics. Please note proxy access for children is not available for children aged 13-17.

When can I see my test results in MyChart?

Results of common lab tests are immediately available in MyChart. Other tests are released 3 or 7 days after finalized , including imaging tests and pathology. Many providers will manually release them earlier, often with explanatory comments.
