There are three easy ways to sign up for MyChart: 1 Register online – You may have been given a 10-digit activation code at your last doctor’s appointment. If so, click here to register online. No need to worry if you don’t have an activation code. You can still register for MyChart by providing a little bit of information about yourself. Click here to register without an activation code. 2 Register at your provider’s office – It’s quick and easy to set up an account at your health care provider’s office during your next appointment. 3 Register by phone – Prefer to register for my chart by phone? How to join the Lourdes portal?
Option 1 – Talk to your Lourdes Provider’s office staff or visit the Hospital Registration Department to receive an email invitation to join the portal. An email invitation will be sent to you with a secure, unique link. Click on the link in your email to complete the registration process. You will need the 4 digit security code decided upon when we invited you to join the portal. To ensure your privacy, you must visit your provider’s office and verify your identity to receive registration information.
