Simply request a MyChart activation code online or during an office visit with Luminis Health Medical Group , Luminis Health Imaging or any other participating practice. Your activation code enables you to log in and create your own user ID and password.
How does Luminis health protect my medical information?
Luminis Health is committed to protecting the confidentiality of your medical information. We limit Luminis Health employees’ access and ability to enter or view information based upon their role in your care.
