What is Lurie Children’s MyChart?

Lurie Children’s MyChart gives you direct online access to portions of your child’s electronic medical record (EMR), where your child’s health information is stored. Test results, appointment information, medications and more are all securely stored for easy retrieval.

What should I know about MyChart?

How do I sign up for Erie MyChart?

Erie will text or email you a sign-up link. With your activation code, go to Erie MyChart at www.myeriehealth.com OR by download the free MyChart app on your phone. Then follow the prompts to login to your account. Patients with an old Erie patient portal account must register for a new portal account.

What to do if your signature cannot be validated?

If your signature cannot be validated, you may be asked to provide supporting documentation that proves your authority to sign the authorization on the behalf of the patient. Please have your signature witnessed.

How long does a mental health release last?

If not otherwise specified, it will expire within 30 days of the date of signature, the exception being mental health releases which expire in one year from the date of signature.

How long does it take to get a health information request?
