MyChart gives you convenient and secure online access to your healthcare record. Whether you’re at home, traveling or working long hours, you can access your medical history, communicate with your doctor, see lab results, set up appointments and more. With MyChart, you can securely access all your health records with a single username and password.

What is mymemorialchart?

myMemorialChart offers you personalized and secure online access to portions of your electronic medical record (EMR), where your provider maintains your health information. It enables you to securely use the internet to help manage and receive information about your health wherever and whenever you need.

How do I get access to MyChart If I don’t have a code?

If you do not have a MYchart account, you will need to request access. Click “Sign up now.” Click “Sign up online” under “No activation code?” and follow the prompts to complete your request. A MYchart invitation email will be sent to you within three to five business days after you submit this request.

How can I view providers’ notes in MyChart?

To help you feel more involved in your child’s care, we have added a feature in MYchart called Open Notes. Through Open Notes, you can now view providers’ notes in MYchart. Notes often cover information about diagnoses, tests, procedures, medicines and treatment plans. The timing for when you can view notes depends upon the care setting.

Your myMemorialChart information comes directly from your electronic medical record at your provider’s office and our hospitals. You can send a myMemorialChart message asking your provider to correct your medical record or call their office directly. You can also have your provider update the information at your next visit.
